ABSTRACT

Objective To evaluate the clinical effect of Phloroglucinol combined with Dexamethasone in ureteroscopy lithotripsy. Methods Patients with severe urinary infection, urinary tract tuberculosis, congenital urinary tract malformation were excluded. The clinical data of 124 patients with ureteral calculus was retrospectively analyzed, Group control with 29 patients who underwent conventional ureteroscopy lithotripsy, 32 patients were given Phloroglucinol (80 mg) as Group phloroglucinol, 29 patients were given Dexamethasone (10 mg) as Group Dexamethasone, 34 patients were given combination drugs of Phloroglucinol (80 mg) plus Dexamethasone (10 mg) as Group combination therapy, intravenously 15 min prior to surgery, respectively. The surgical complication, side-effects, operation time, duration of postoperative hematuria, and expulsion rate were recorded. Results All surgeries were accomplished successfully, and none severe side-effects were observed among these cases. In comparison of operation time and duration of postoperative hematuria, there were no significant difference between group Dexamethasone and group control, and both group Phloroglucinol and group combination therapy were better than that of group control (P < 0.05), furthermore, group combination therapy was better than group phloroglucinol (P < 0.05). In comparison of the expulsion rate, there were no significant difference between group control and group Phloroglucinol or group Dexamethasone (P > 0.05), however, group combination therapy is better than that of group control (P = 0.017). Conclusion Combined treatment with Phloroglucinol and Dexamethasone in ureteroscopy lithotripsy would shorten the operation time and duration of postoperative hematuria significantly, and improve the expulsion rate eventually.

ABSTRACT

A method of identification of C=C location and relative quantitation of unsaturated phosphatidylcholine ( PC ) isomers in breast cells by online photochemical reaction-pulsed directed current electrospray-tandem mass spectrometry ( PB-pulsed-dc-ESI-MS/MS) was established with benzophenone ( BP) as a photochemical reactant. The three-phase extraction method was used to extract the lipids in the cells, and then the C=C in the unsaturated PC and the carbonyl in BP were specifically cycled under the irradiation of 254 nm ultraviolet light (Paternò-Büchi, PB reaction). The PB products were ionized and mass-isolated for low-energy collision dissociation through the non-contact pulsed-dc-ESI ionization method. The double bond position and the relative content of the location isomers were obtained from the resulting ions in the MS/MS spectrum. The C=C location of 8 kinds of unsaturated PCs in MCF-7 and MCF-10A was detected, and the relative contents of 4 kinds of C=C location isoforms were analyzed. It was found that the relative abundance of △9 isomer in PC 16:018:1 was not significantly different between the two cells. The relative abundance of△9 isomers in PC 18:018:1 and PC 18:118:1 was slightly different. However, there is a big difference of △9 in LPC 18:1 between the cancer cell and normal cell (56. 0% ± 1. 3% vs. 71. 7% ± 6. 8%). The establishment of such a rapid and easy mass spectrometry method can analyze the C=C location and the relative content of location isomers, and it is expected to be a powerful tool to identify different cell states and different disease states.

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect and acting mechanism of puerarin preconditioning (PP) on blood level of cytokines in patients undergoing cardiopulmonary bypass (CPB) in perioperative period.</p><p><b>METHODS</b>Forty patients with heart diseases scheduled to take surgical operation were randomized into the control group and the PP group equally. They were treated with the same program, excepting that 0.6 g of puerarin was given to the PP group by adding in 5% glucose solution 250 mL for intravenous dripping every day for one week before operation, but to the control group, normal saline was given instead. The levels of tumor necrosis factor-alpha (TNF-alpha), interleukin 6, 8 and 10 (IL-6, IL-8, IL-10) in arterial blood were measured at 5 time points in the process of CPB, namely, anesthetic induction (T1), 10 min after the clamp of the ascending aorta (T2), 10 min, 2 h and 12 h after the Clamped aorta is unclamped (T3, T4 and T5).</p><p><b>RESULTS</b>All the above-mentioned indexes (TNF-alpha, IL-6, IL-8 and IL-10) gradually increased after beginning CPB, reached the peak at T4, then lowered gradually but still presented the higher levels at T5 than those at T1 (P < 0.05). Comparison between the two groups showed that levels of TNF-alpha, IL-6 and IL-8 were significantly lower (P < 0.05 or P < 0.01) and level of IL-10 was higher in the PP group than those in the control group respectively at all the time points (P < 0.01).</p><p><b>CONCLUSION</b>Injecting puerarin before CPB could effectively suppress the pro-inflammatory cytokines like TNF-alpha, IL-6 and IL-8; and enhance the expression of anti-inflammatory cytokines like IL-10, thus to alleviate the inflammatory reaction induced by CPB.</p>
